Annette Sharpe

Support Coordinator - Port Macquarie

Annette comes to See Changes with a background in both home care and disability services. Her most recent experience has been in supporting NDIS clients as a Support Coordinator. Annette is passionate about assisting participants to navigate their NDIS Plan, connecting them with the best providers suited to their needs, and providing person-centred supports so that they have the best chance of reaching their goals. In her spare time, Annette loves spending time with her adult kids, particularly attending live concerts.


Chantal Campbell

Systems Support Manager

Chantal commenced with See Changes in April 2024. She comes to this role with over 10 years’ experience in the disability sector across a variety of roles. Chantal is passionate about working collaboratively with individuals to help them improve their lives and achieve their goals. When she’s not working, Chantal enjoys family time with her two children and their dog, Fern. They love their beach time.


Cheryl Earley

Support Coordinator – Taree

Cheryl brings to See Changes many years of experience working in community. Initially working in accommodation services as a Disability Support Worker, Group Home Team Leader, and Manager, Cheryl stepped into the NDIS Support Coordination role in 2018. Cheryl says “ I found it to be the most rewarding job I had ever done”. Cheryl is tireless in her support of NDIS participants, ensuring that they have a voice and are included in the choice of services they need to support their individual goals. Cheryl has a person-centered , holistic work ethic, and when not doing this important work she likes to potter in her garden and build on her collection of succulents. Cheryl also enjoys her volunteer work as a committee member of the Taree and District Eisteddfod for the past 13 years and enjoys hanging out with friends and family, in particular her two little grandchildren.


Elizabeth Patullo

Behaviour Support Practitioner - Port Macquarie

Elizabeth is a Behaviour Support Practitioner and Holistic Integrated Creative Arts Therapy Practitioner located in Port Macquarie. Elizabeth prides herself on delivering empathetic, person-centred support. Elizabeth holds a Bachelor of Psychology (honours) degree and has extensive experience within early childhood education and corporate settings. Elizabeth is passionate about empowering people of all ages to achieve their goals through behavioural and creative processes that enhance physical, mental, and emotional wellbeing. Outside of work she enjoys spending time with her family, being out in the garden or at the beach.


Erin Schubert

Behaviour Support Practitioner - Newcastle

Erin graduated from the University of Newcastle with a Bachelor of Psychological Science (Honours) in 2021 and has been working in the NDIS space since 2019. She comes to See Changes with experience in disability advocacy and support work, crisis management and, more recently, behaviour support where she has decided to build her career. Erin enjoys collaborating with allied health professionals to provide comprehensive supports to her clients. She is experienced with working with people of all ages and presenting with a variety of psychosocial, intellectual, developmental, and physical disabilities. In her spare time, Erin enjoys spending time with her partner and her dog, Penny, as well as going on walks and visiting local markets.


Gabriel Brearley

Systems Support Officer

Gabriel began working with for See Changes as a Systems Support Officer in September, 2024. Gabriel has a broad background in hospitality, entertainment and accommodation, both in “hands on” and administrative roles. He is passionate about achieving client satisfaction and assisting in running business operations smoothly and efficiently. Gabriel loves spending his spare time with his partner, his cats and playing recreational sports and games.


Jamie Boulton 

Behaviour Support Practitioner - Taree

Jamie is an experienced Behaviour Support Practitioner based in Taree. She comes to us with 15 years of experience across various roles in the disability industry including support work, support coordination and, most recently, behaviour support. These roles have helped Jamie develop skills in supporting individuals with ASD, ADHD, Complex Trauma, ABI, psychosocial disabilities, and intellectual disability. She has also supported individuals in contact with the criminal justice system, young people in out of home care, and older persons in aged care. Jamie is in the final stages of her Bachelors degree in Social Welfare. In addition to this, she hold Cert IV in Disability, Mental Health, Drug and Alcohol, and Community Services. In her spare time she loves spending time with her dog, Luna, and competes in state darts events. 


Jodie Aston

Support Coordinator - Port Macquarie / Central Coast

Jodie comes to See Changes with an extensive 25-year career in the disability and aged care sectors. Over this time, she has had to work across a myriad of different funding models, and she’s up there with the best in the business in her understanding of NDIS funding. Jodie has been a Support Coordinator for 10 years and is an experienced Psychosocial Recovery Coach. Jodie works diligently to support her clients to connect with NDIS and mainstream providers, as well as government services. Her aim is to connect participants with the services most suited to their needs. Jodie also prides herself on supporting her participants to understand their NDIS Plans and to pursue their goals so that they can lives their best lives.

Jodie is very family oriented. She is happily married, with two adult children and an amazing granddaughter with whom she can’t spend enough time. She also enjoys living by and kayaking on the river, as well as looking after all of their farm animals.


Kara Vallins

Behaviour Support Practitioner – Newcastle

Kara joined See Changes in March 2025, bringing experience in behaviour support and counselling. She is passionate about creating positive change and values collaboration, always striving to work alongside individuals, families, and teams to achieve the best outcomes. Kara brings a mental health lens to her behaviour support work and takes a thoughtful approach to assessments and the development of behaviour support strategies, ensuring they are tailored to each person’s unique needs. Outside of work, Kara enjoys spending quality time with her partner and friends, staying active at the gym, and exploring the beautiful local beaches around Newcastle.


Karen Ford

Behaviour Support Practitioner – Singleton

Karen Ford brings over 20 years of dedicated experience within the disability and welfare sectors, including more than 8 years as a Behaviour Support Practitioner. Her background in residential support provides a valuable foundation for her behaviour support work. Karen is known for her person-centred and holistic approach, demonstrating a strong commitment to supporting participants in enhancing their overall wellbeing. She has experience working with a diverse range of cohorts including individuals with intellectual disability, autism, acquired brain injury (ABI), mental health conditions, and alcohol and other drug (AOD) dependencies. Karen is recognised for delivering supports that are practical and achievable. She has always had a respect for and interest in the diversity of culture, personal interests and the strengths in goal setting that can be achieved by inclusion in the tapestry of life. In her personal life, Karen is interested in art, ancient history, backgammon, board games, waterfalls and gardens.


Kate Brearley

Managing Director

Kate's senior experience spans over 20 years in disability services. She's not just our founder; she's the driving force behind See Changes. Her passion and dedication blends with decades of industry experience in program coordination and operational management – underpinned with a Bachelor of Arts in Psychology and diplomas in Leadership and Management. Kate's approach is all about putting people first – ensuring every service at See Changes is tailored to the unique needs of the community across all areas of NSW. Her mission? To empower lives through exceptional in-home therapies delivered by exceptional people.


Kelly Wright

Behaviour Support Practitioner – Newcastle

Kelly joins See Changes as a Behaviour Support Practitioner with a rich background in Social Work and Education. She brings extensive experience working with individuals and families. Kelly is highly skilled in assessment and therapeutic support, individual and family counselling, and working with diverse populations including those with mental health issues, disabilities and neurodivergence. Her expertise also extends to supporting new parents and those dealing with alcohol and other drug issues. Kelly is trauma-focused, ensuring that her approach is sensitive and tailored to the needs of those she supports. Her dedication and comprehensive skill set make her a valuable asset to the See Changes team. In her spare time, Kelly can most often be found with her 5yr old, husband and Spoodle wandering in nature.


Kristin Vella

Senior Occupational Therapist (Consultant) - Newcastle

Kristin is an Occupational Therapist who graduated from the University of Newcastle in 2011. Over the course of the last 14 years, Kristin has worked in vocational rehabilitation, acute care (stroke/neuro), inpatient rehabilitation (stroke) and community settings in Australia, the UK and New Zealand. Kristin has a special interest and experience in working with adult clients in their homes, to complete functional capacity assessments, activities of daily living skill building, assessment and prescription of aids and equipment, home environment assessments/falls risk support and pressure care. Additionally, Kristin has worked for the last few years in the NDIS space with people experiencing psychosocial disability, to problem solving to find meaningful goals to work towards, support with improving their daily living skills and routines, sensory and regulation strategies and improve connection with their communities. Kristin really enjoys working in the community, with people in their homes. She values education and believes that no lasting change can be made without fully understanding the problem and the range of solutions that could assist. Kristin is a strong communicator who supports an individual’s autonomy; she is strengths and solutions focussed and always aims to help individuals live their best life.


Margaret Trzcinka

Speech Pathologist - Newcastle

Margaret joined See Changes in May 2024, bringing with her over 20 years of experience in both government and non-government settings. Margaret has literally supported hundreds of individuals across her career and has tons of experience working with individuals with intellectual and developmental disability, neurological and neurodegenerative conditions, as well as acquired / traumatic brain injury. She is highly experienced in supporting individuals with both swallowing and communication issues. In her spare time, Margaret loves spending time with her family, watching movies and documentaries, as well as enhancing her KWS signing skills.


Sam Gaynor

Senior Behaviour Support Practitioner - Central Coast

Sam Gaynor is a Central Coast local who has worked in the Disability & Mental Health sector for 20 years. Sam started off as a Support Worker where he quickly developed a keen interest in working with those that exhibited behaviours of concern, working his way up in a number of operational roles. In 2011 Sam applied to become a Behaviour Support Practitioner while working in Western Australia under a new government initiative, to which he was successful, and spent the next two years being trained by Dr Geoffrey Potter in all things Positive Behaviour Support (PBS). Besides this, Sam holds a Bachelor Degree in Social Science & a Masters of Social Work. 

 

Since then, Sam has worked in leadership role positions such as Clinical Manager, Practice Lead (National) & Principal Practitioner of Behaviour Support (National). Sam has made the decision to jump back in as a Senior Behaviour Support Practitioner for two reasons: to do what he really loves, that is, to work closely with participants & families; and secondly, to spend more time with his own family, taking a role that offers more flexibility.


Victoria Refalo

Manager / Behaviour Support Practitioner – Maitland

Victoria's dedication to strengths-based support shines through her work. With a foundation in supporting children and young people in Out Of Home Care, Victoria has honed her skills in addressing a broad spectrum of diagnoses and complexities stemming from trauma. Now supporting participants of all ages, she is a registered Behaviour Support Practitioner with the NDIS. Her academic background includes a Bachelor of Education (Secondary) paired with a Bachelor of Arts, and she is currently advancing her expertise through a Masters of Applied Behaviour Analysis.
